It all started when I said something stupid in school. A girl was ignoring me, and I got mad and said, “F-ck this sh-t. I’m gonna do some Virginia Tech sh-t.” I only said it so the girl would pay attention to me. But I shocked all my classmates and teachers, and the school said I’d made a “terrorist threat.”

I was in the 9th grade, and I had recently moved out of an abusive situation with my mom and into a foster home I knew nothing about. I needed someone to listen so I could get my feelings out. But there was no one I could really trust.

My caseworker came to my foster mom’s house and told me that he would take me to KFC and then to a “nice place to get help.” I thought, “OK, that sounds cool. I get my favorite food and I go to a center to feel better.”

The next stop we made was a psychiatric hospital for kids. We went through door after door, and it dawned on me that every door had a lock. Once the door shut you couldn’t open it. The doors locked you in. They intended to keep me here. That realization gave me a panic attack. I started running and the security tackled me. I was forcibly dragged in.

What Was I Signing?

When I got inside, the kids peeked out of their rooms to see who was coming. I was so scared I thought I would pee on myself. I had never been to a place like this. When I entered a dayroom, a place where the kids hang out, they slowly introduced themselves. I shook my head in fright. I wasn’t like these kids. Some were twitching and others drooled. I kept to myself and didn’t speak a word to anyone.

I felt forced into signing a bunch of papers. I didn’t realize I was signing consent to take medication.

The first things they prescribed were Depakote and Risperdal. I didn’t get a say in what I wanted, and that made me feel powerless.

At the hospital, staff joked about it in a perverse way. “Hey kids, come and get your happy pills!” “Come right up for your Skittles, it makes the world a better place!” I was disgusted that the staff were making light of my situation. I wondered how they’d feel if they were forced to take pills in a lockdown facility.

The meds made me feel bad. Sometimes I over-ate, ate too little, or had trouble sleeping. I hated the fake smile the nurses gave me after I took my medication.

I didn’t want to talk to anyone, especially my therapist, because I believed that my depressing stories about my mom’s abuse might make the doctors prescribe more medication.

I was afraid if I kept taking medication I would be just like every kid in the hospital. I wanted to be the kid who stood out, the kid who didn’t take medication. There were kids already looking up to me but I wanted them to think, “Wow, Anthony doesn’t take medication. I want to follow his lead.”

I tried hiding the pills in my hand. I learned how to put pills deep in my throat and spit them out later. It worked for a while but then one pill got stuck there. The staff helped get it out. After that they checked me carefully.

Another way I avoided pills was simply putting them under my tongue. I would hide them in a soap bar box until my roommate saw it and told the nurse. Then I was forced to take liquid medication, which was disgusting.

A Target

The Depakote was supposed to make me feel “calmer” and “happy.” Instead I gained over 30 pounds, and that brought my self-esteem down. I felt fat and I wasn’t comfortable with myself. Some of the kids and even staff called me names like fat ass or b-tch tits. I went off on one staff once because he said, “I know the perfect birthday present for you—a training bra!”

I really wanted to do well, and I tried to behave and present myself in a mature manner. But it didn’t seem to make a difference. And the uncontrollable and unpredictable behavior around me started to affect me.

The one and only time I truly flipped out, though, was when the whole unit tried to jump me. “Yo, let’s f-ck up this p-ssy n-gga Anthony,” said one kid. Suddenly everyone turned to me grinning sinisterly, like they’d just found their new target.

“Nah, come on guys, let’s play some board games or something,” I suggested.

“You ain’t gonna get out this, b-tch,” said a fat kid with squinty eyes. “You think you Mr. Goody Two Shoes. We gonna straighten you out.”

I ended up getting chased down by 12 guys. One person caught me and then they stomped me out. I thought I would beg for them to leave me alone, but suddenly I felt myself becoming so enraged that I no longer felt the pain. I got up and screamed, “LEAVE ME ALONE!!!”

I was surprised at my sudden outburst, but most of the guys just laughed. Then everything turned red and my surroundings became a blur. I didn’t gain full consciousness until I was near the dayroom area. I noticed some of the guys holding their lip or arm. “Did I do this?” was the only thought that came to mind.

I was shocked that I’d stood up to them, much less beaten them up. A weird feeling came over me then. I wondered for the first time in the hospital if I was losing my sanity and just becoming one of maybe thousands of nut jobs who end up staying in hospitals.

Suppressing My Feelings

But most of the time I was quick to disengage and try to find ways to occupy myself when I saw these kinds of incidents starting. I tried reading, writing, talking with a staff I could trust, or daydreaming. These were ways to block out any negativity that surrounded me. Although these strategies were very helpful, I was still suppressing my feelings because there were overwhelming situations I wasn’t familiar with and didn’t know how to deal with emotionally.

While I was in the hospital, I saw two people commit suicide, including my roommate. They said I was “further traumatized” by that and put me in a state hospital, which was even more restrictive.

Looking at it now, I can see that the suicides did really impact me. However, I felt outpatient therapy (therapy where you see your therapist but you’re not confined to a psychiatric unit) could’ve been more effective. I didn’t see how living in the state hospital was going to help. I just wanted to be back in the community where I’d be able to interact more freely, go out, and feel more like a normal kid.

I was glad to leave the first hospital, but this was no better. I wanted to get off medication completely. Some doctors finally decided I was stable enough to behave without meds. They started to take me off a little at a time. I was happy to be off the medication, but if I messed up or acted out one bit, like by cursing, I was back on it.

For example, once a staff ticked me off by yelling at me for not doing my laundry. I cursed at him because he kept pressuring me. The doctors and staff said the fact that I cursed meant I was too unstable to stay off medication. But wouldn’t anyone curse if they felt pressured or nervous that a staff he hardly knew started yelling at him?

I had seen some staff do terrible, abusive things to the kids, like getting them to fight each other in exchange for Chinese food (a special treat). Of course I was on edge around some of the staff. The doctors didn’t know that, though.

Can’t We Talk About This?

I felt trapped. Some doctors said, “Well, Anthony, it’s possible to get off medication, but will it benefit you in the long run?” What were they trying to say? That I couldn’t function properly without the use of a drug?

I didn’t question it further because the mental health system had trained my brain to think that meds were my solution to everything. If I felt angry the doctor would say, “Maybe it’s time for Abilify, a drug that stabilizes your mood swings.” If I felt anxious the doctor would try to prescribe Zoloft, a pill that helps with some types of anxiety. I thought, “Have you guys ever heard of talking your feelings out? NOT EVERYTHING CAN BE SOLVED WITH THE USE OF A DRUG!”

I was receiving therapy at the time, and I felt it helped more than the meds. I had a really good therapist, and it was such a physical release to be able to express my feelings. I’m sure the meds did improve my moods somewhat; I was less likely to curse and talk back. But what helped the most was having a direct connection with a trusted adult like I got in therapy.

I sat down one day and wrote how I felt the pills were helping me—pros—and how they weren’t—the cons. I wanted time to reflect on where I was going in life, to feel some control. The cons on my list—the physical side effects, and the depressing feeling I got from taking meds—outnumbered the pros. I wasn’t going to tell the doctor that everything I was taking was all right with me. It wasn’t and I had to put a stop to it.

I was tired of taking meds and then being taken off just to get back on again. No one even gave me a real explanation. Their excuse was usually, “We’re putting you back on because we feel you could be in a more stable condition.” Being on and off meds made me really jumpy. My eyes would twitch sometimes.

I also felt mentally tired because I’d been on drugs for over a year and I wasn’t getting better. I was constantly sleeping and I couldn’t focus. Emotionally, I was tired of the need to even be on meds in the first place.

I believed that in order for me to be better I had to be exposed to the community because then I could feel how a teenage life is supposed to be. To me this meant a cell phone so I could communicate with friends, my own room, decent curfews, a real home, and to be around my family. It wasn’t pills I needed; it was the chance to feel like a normal teenager after years of abuse and being institutionalized.

Love Is the Best Medicine

After eight months at the second hospital, I was sent to a group home at a Residential Treatment Facility (RTF), where I continued to take medication. I began to wonder when I would ever get back in the community. I had just started going on visits with my aunt and I had decided that I would like to go live there. I just wanted to stay somewhere permanently and feel cared for. Thinking about all this moving made me as depressed as when I first came into the hospital.

Finally, they let me go live at my aunt’s house. I think the reason why the RTF agreed to it was because I kept advocating for myself. I felt excited and at peace. I felt that I had achieved the impossible and that I deserved to be with my aunt and my family who would love me for me, instead of living with the institution’s idea of “support.” I had worked two and a half years to get to this point. I would not let it go to waste.

Alone in my room at my aunt’s house, I thought quietly. I looked to the left. There was no nurse ready to give me a cup full of meds. I looked to my right. There was no doctor trying to switch my meds or giving me higher doses. It dawned on me then. There were obviously rules and expectations, but ultimately I could make my own decisions now. I didn’t have to continue the medication. So I made an appointment with the doctor and said, “I no longer feel like I need medication.”

The doctor seemed a little concerned that I was in a rush. She said, “Anthony, you’re a very bright kid, but are you sure that you want to get off? I want you to perform at your highest and do well.” I told her I was sure of my choice and that I wouldn’t regret it. And I don’t.

The Community Transformed Me

Now that I don’t take medication I feel a lot happier, more powerful, and in control. Yeah, I had to get adjusted to living back in Brooklyn, but I adapted quickly. It felt good to see my neighborhood friends and the employees I always talked to at the Burger King across the street. I never ever felt this happy when I was on medication. I always felt drugged or out of it. I’m not always happy, but when I do feel bad I talk my feelings out with people I trust, and I write. Writing allows me to get overwhelming or negative things off my mind onto paper.

Being in the community is what I’ve always wanted. Now I have a sense of freedom. I go to regular school, I have easy access to friends, and I socialize on my time. I’m not on someone else’s schedule and I don’t have to be cooped up inside all day feeling anxious. The community has transformed me.

So far, opposition to DSM 5 has been expressed by the following organizations: British Psychological Society; American Counseling Association; Society for Humanistic Psychology (APA Division 32); Society for Community Research and Action: Division of Community Psychology (APA Division 27); Society for Group Psychology & Psychotherapy (APA Division 49); Developmental Psychology (APA Division 7); UK Council for Psychotherapy; Association for Women in Psychology; Constructivist Psychology Network; Society for Descriptive Psychology; and the Society of Indian Psychologists.

An editorial by the Society Of Biological Psychiatrywondered whether DSM 5 was necessary at all. The community of personality disordersresearchers is virtually unanimous in its opposition to the DSM 5 personality disorders section. There has also been widespread opposition to the sections on somatic, autistic, gender, paraphilic, and psychotic disorders.

Last week, a petition was posted quietly be several divisions of the American Psychological Association. It demands reform of the DSM 5 process and the elimination of a number of its most risky and ill conceived proposals. The petition is gaining increasing support and has already been signed by almost 3000 people. It can be accessed at http://www.ipetitions.com/petition/dsm5/ )

Strikingly, there seems to be virtually no support for DSM 5 outside the very narrow circle of the several hundred experts who have created it and the leadership of the American Psychiatric Association (APA) which stands to reap large profits from its publication. There is no group and precious few individuals outside of APA who have anything good to say about DSM 5. And even within the DSM 5 work groups and the APA governance structures, there is widespread discontent with the process and considerable disagreement about the product.

It is the tale that launched a thousand alter egos: the famous true story of “Sybil”, who endured years of torture at the hands of her sadistic mother and grew up into the meek, anxiety-ridden adult whose head was said to house 16 personalities.

For many, she provided a startling introduction to a rare and intriguing condition: then known as multiple personality disorder (MPD), a disease of the mind affecting mostly women, in which a person hosts several vastly different personalities representing fractured aspects of a haunted past.

Luckily, with the help of her psychiatrist’s enduring dedication to her treatment – which included many punched-out office windows and late-night house calls – Sybil was finally able to come to terms with the other sides of herself and integrate them, triumphing over her disease. The tale made for a compelling book, Broadway show and an even more engaging movie in 1976 (and a less riveting remake in 2007). The book and film became instant classics, not to mention teaching tools for psychology students.

But according to investigative journalist Debbie Nathan, the story of Sybil has one big problem: it’s mostly bunk.

In Sybil Exposed, Nathan, famous for her exposés on “recovered memory syndrome”, goes through the story, claim by claim, with a fine-toothed comb. It’s a massive undertaking of research that teases apart fact from fiction to reveal an even more interesting and educational account of, not 16, but just three personalities: the author, Flora Schreiber, the psychiatrist, Cornelia Wilbur, and “Sybil”, Shirley Mason.

What Nathan found among the archives was “shocking but utterly absorbing”, she says. Mason’s 16 personalities had not appeared spontaneously as they do in the book and movie, but were “provoked over many years of rogue treatment that violated practically every ethical standard of practice for mental health practitioners”, she writes.

This is quite a firebomb to throw into a heated battle that started in the late 1990s and is still being fought today. The lines are drawn between whether MPD, since renamed dissociative identity disorder, exists as an artefact of post-traumatic stress disorder, as its own unique illness, or if it is merely the product of wishful, reinforcing therapy and willing clientele.

While the next edition of psychiatry’s bible, the Diagnostic and Statistical Manual of Mental Disorders, is in development, this is no small quibble.

Nathan uses direct quotes from the actual psychoanalysis session transcripts, excerpts from Mason’s diaries, and Schreiber’s author notes to provide fascinating insights into how these three women turned sickness and desire into a business.

When Wilbur, an ambitious female psychiatrist in a field packed with men, found Mason, an attention-starved and admiring patient, it was not long before they were engaged in a twisted parent-child kind of relationship. Nathan shows how Wilbur supplied her patient with attention and affection, and Mason eagerly performed whatever dance seemed to please Wilbur most.

At one point Mason wrote a letter attempting to confess to Wilbur that she had invented these personalities. She also asked that they stop “demonizing” her mother – who Wilbur had cast as a vicious abuser but who Nathan suggests was just religiously strict and emotionally unpredictable. Wilbur dismissed the letter as a defence mechanism, and her patient, desperate not to lose the doctor’s interest, continued the charade. Soon after, the two women met Schreiber, who spun their story into the profitable and sexy legend we know today.

Sybil remains a good book and movie, but perhaps Nathan’s version of the story is the one worth telling in classrooms. Though it is the less sensational tale, it is a cautionary one. It is a solemn reminder of why mistrust plagues the mental health field, and why we must always be careful to pin down the facts, and leave it to fiction to get carried away with the story.

SERIOUS problems in Victoria’s mental health system have been revealed recently in The Age. The important thing now is to find solutions. In doing so we should remember that although Victoria is in the spotlight, similar ”crises” occur regularly all over the world. Perhaps this is because Victoria is not alone in having a system based on fundamentally flawed principles.

Mental health services have become increasingly dominated by psychiatry’s ”medical model”, which claims that feeling depressed, anxious or paranoid is primarily caused by genetic predispositions and chemical imbalances.

This has led to alarming rises in chemical solutions to distress. In New Zealand, one in nine adults (and one in five women) is prescribed antidepressants every year.

The public, however, in every country studied, including Australia, believes that mental health problems are caused by issues such as stress, poverty and isolation. The public also prefers talking therapies to drugs and electroconvulsive therapy (ECT).

Research suggests the public is right. For example, the single best predictor of just about every mental health problem is poverty, followed by other social factors such as abuse, neglect and early loss of parents in childhood, and – once in adulthood – loneliness and a range of adverse events including losses and defeats of various kinds.

Meanwhile, reviews of studies on anti-depressants (which only recently have been able to include those previously kept secret by drug companies) conclude that they are superior to placebos only for those at the extreme end of the ”most severe” group of depressed people. This represents less than 10 per cent of the people who are receiving these drugs.

A recent Cochrane review (the type most highly regarded in the scientific community) for risperidone, a leading anti-psychotic drug, ”suggests that there is no clear difference between risperidone and [a] placebo”.

A placebo (from the Latin meaning ”I please”) is not necessarily a bad thing. Indeed the talking therapies are effective partly because, if done well, they too instil hope and expectations of recovery.

The problem is that psychiatric drugs often have serious adverse effects. Anti-psychotics, for instance, can cause rapid weight gain, loss of sexual function, diabetes, heart disease, neurodegeneration and reduced life span.

As previously reported, my review of ECT studies (with Professor Richard Bentall of Liverpool University) found that this treatment is ineffective for most recipients and frequently causes permanent memory loss. This in itself can be depressing.

ECT also has a slight but significant risk of death, most frequently from cardiovascular failure.

Inpatient units are equally ineffective and can also be damaging. When will we learn that putting large numbers of extremely distressed people in the same building is not a good idea?

What I conclude from all this is that any review of mental health services in Victoria, or anywhere else for that matter, should probably be led by anyone other than a psychiatrist – and certainly not in Victoria’s case the state’s Chief Psychiatrist, whose job, according to Dr Ruth Vine herself, is “to watch over how the system is functioning”.

It is unfair to expect Dr Vine to take an objective view on the failure of the system for which she is responsible. That lack of objectivity is amply demonstrated by her claims that ECT is “safe and effective” and that the problem is the public’s “negative” views.

Perhaps a lawyer from the Mental Health Legal Centre might be a good choice.

Federal agents have arrested dozens of suspects charged with bilking Medicare of hundreds of millions of dollars in bogus services for mental health therapy and other types of healthcare.

Agents with Health and Human Services and the FBI have fanned out across three South Florida counties, arresting clinic owners, healthcare employees, patient recruiters and assisted living facility owners who allegedly supplied hundreds of patients to the mental health clinics.

The sweep comes after the indictment of Miami-based American Therapeutic Corp., which was charged along with 24 employees and others over the past year. That case alone involved $200 million in false claims submitted to the federal healthcare program for the elderly and the poor.

American Therapeutic’s top executives and others have been convicted in recent months. The latest sweep entails clinics offering group therapy sessions, home healthcare, HIV services and medical equipment.

The U.S. attorney’s office is expected to have a news conference Wednesday afternoon to provide details of the cases and defendants.

In recent years, Senator Grassley exposed several academic physicians for taking large amounts of money from companies with a direct financial interest in their research, some of it funded by the NIH. The list reads like a who’s who in academic research:

• Dr. Charlie Nemeroff, former Chair of the Psychiatry Department at Emory University, who failed to report hundreds of thousands of dollars in payments from GlaxoSmithKline while researching that same company’s drugs with an NIH grant.  Dr. Nemeroff was bounced from Emory and has now taken over the Chair of Psychiatry at the University of Miami.

• Dr. Alan Schatzberg, former Chair of the Psychiatry Department at Stanford University received an NIH grant to study a drug while partially owning a company that was seeking FDA approval of said drug. An NIH oversight group recommended that Stanford’s clinical trial on mifepristone be “terminated immediately and permanently.”  The recommendation was made because of concerns over conflicts of interest, patient safety and other issues.
• Dr. Joseph Biederman and two other researchers at Harvard University failed to report almost a million dollars each in outside income while heading up several NIH grants. Harvard later disciplined the three physicians.

US health agency revises conflict of interest rules

WASHINGTON,  – The U.S. National Institutes of Health revised on Tuesday its 16-year-old conflict of interest rules for medical researchers, lowering the amount of money that constitutes a financial conflict and expanding the required disclosures.

The 1995 regulations effectively put responsibility for tracking scientists’ financial conflicts of interest on their universities. The rule required researchers to disclose conflicts to their institutions, which then had to assure the NIH the conflict had been managed, reduced or eliminated.

The new rule extends that requirement so researchers report not only the fact of a conflict of interest, but also its details such as value, specific nature, why it is a conflict and the impact it might have on research.

It lowers the amount a researcher must disclose if received from an industry or held in company stock to $5,000 from about $10,000.

Research institutions, in turn, are now required to report that information to the federal grant-awarding agency alongside details of how the conflicts are managed. Also, before spending any grant money, the institution has to post information about the financial conflicts on a public website.

The new rules will affect about 2,000 organizations that are awarded public health science funding every year and some 38,000 scientists who participate in research funded by these grants and have a “significant financial interest,” NIH said.

Concern about the integrity of research in the United States has grown since 2008, when Iowa Republican Sen. Charles Grassley criticized prominent Harvard University psychiatrist Dr. Joseph Biederman and others for failing to fully disclose payments from drug companies.

In a more recent example, medical device maker Medtronic Inc (MDT.N) came under fire over accusations that doctors paid by the company had failed to disclose major side effects from a bone growth drug in clinical trials.

A 2009 report by the Institute of Medicine, one of the National Academies of Sciences that advises U.S. policymakers, called on doctors to strictly disclose research funding to strengthen protection against conflicts of interest. The report called for virtually anyone involved in medicine — academic medical centers, journals, professional societies, researchers and doctors — to set up or strengthen conflict of interest guidelines.

From 1996 to 2007, relationships between individual academic researchers and industry nearly doubled, according to a study cited by NIH in its final rule. From 1994 to 2003, the amount of financial support for biomedical research almost tripled to $94.3 billion, with 57 percent of that funding coming from industry sources, according to analysis cited by NIH. Congressman Ron Paul has re-introduced  The Parental Consent Act ,  A bill which prohibits federal funds from being used to establish or implement any universal or mandatory mental health, psychiatric, or socioemotional screening program.

Yet, parents are already being threatened with child abuse charges if they resist efforts to drug their children. Bill information:  The Parental Consent Act 2011 (H.R. 2769 – previously H.R. 2218  in 2009) Prohibits federal education funds from being used to pay any local educational agency or other instrument of government that uses the refusal of a parent or legal guardian to provide consent to mental health screening as the basis of a charge of child abuse, child neglect, medical neglect, or education neglect until the agency or instrument demonstrates that it is no longer using such refusal as a basis of such charge.

Defines a screening program under this Act as any mental health screening program in which a set of individuals is automatically screened without regard to whether there was a prior indication of a need for mental health treatment, including: (1) any program of state incentive grants to implement recommendations in the July 2003 report of the New Freedom Commission on Mental Health, the State Early Childhood Comprehensive System, grants for TeenScreen, and the Foundations for Learning Grants; and (2) any student mental health screening program that allows mental health screening of individuals under 18 years of age without the express, written, voluntary, informed consent of the parent or legal guardian of the individual involved.

Madam Speaker, I rise to introduce the Parental Consent Act. This bill forbids Federal funds from being used for any universal or mandatory mental-health screening of students without the express, written, voluntary, informed consent of their parents or legal guardians. This bill protects the fundamental right of parents to direct and control the upbringing and education of their children.

The New Freedom Commission on Mental Health has recommended that the federal and state governments work toward the implementation of a comprehensive system of mental-health screening for all Americans. The commission recommends that universal or mandatory mental-health screening first be implemented in public schools as a prelude to expanding it to the general public. However, neither the commission’s report nor any related mental-health screening proposal requires parental consent before a child is subjected to mental-health screening. Federally-funded universal or mandatory mental-health screening in schools without parental consent could lead to labeling more children as “ADD” or “hyperactive” and thus force more children to take psychotropic drugs, such as Ritalin, against their parents’ wishes.

Already, too many children are suffering from being prescribed psychotropic drugs for nothing more than children’s typical rambunctious behavior. According to Medco Health Solutions, more than 2.2 million children are receiving more than one psychotropic drug at one time. In fact, according to Medico Trends, in 2003, total spending on psychiatric drugs for children exceeded spending on antibiotics or asthma medication.

Many children have suffered harmful side effects from using psychotropic drugs. Some of the possible side effects include mania, violence, dependence, and weight gain. Yet, parents are already being threatened with child abuse charges if they resist efforts to drug their children. Imagine how much easier it will be to drug children against their parents’ wishes if a federally-funded mental-health screener makes the recommendation.

Universal or mandatory mental-health screening could also provide a justification for stigmatizing children from families that support traditional values. Even the authors of mental-health diagnosis manuals admit that mental-health diagnoses are subjective and based on social constructions. Therefore, it is all too easy for a psychiatrist to label a person’s disagreement with the psychiatrist’s political beliefs a mental disorder. For example, a federally-funded school violence prevention program lists “intolerance” as a mental problem that may lead to school violence. Because “intolerance” is often a code word for believing in traditional values, children who share their parents’ values could be labeled as having mental problems and a risk of causing violence. If the mandatory mental-health screening program applies to adults, everyone who believes in traditional values could have his or her beliefs stigmatized as a sign of a mental disorder. Taxpayer dollars should not support programs that may label those who adhere to traditional values as having a “mental disorder.”

Madam Speaker, universal or mandatory mental-health screening threatens to undermine parents’ right to raise their children as the parents see fit. Forced mental-health screening could also endanger the health of children by leading to more children being improperly placed on psychotropic drugs, such as Ritalin, or stigmatized as “mentally ill” or a risk of causing violence because they adhere to traditional values. Congress has a responsibility to the nation’s parents and children to stop this from happening. I, therefore, urge my colleagues to cosponsor the Parental Consent Act.

Harvard Psychiatrists Disciplined for Conflicts of Interest

Psychiatrist Joseph Biederman was funded millions by Pharma while promoting child "bipolar" disorder

The primary promoters–inventors, one might say– of diagnosing children with “bipolar” disorder, who for over a decade, aggressively promoted the biopolar diagnosis and use of antipsychotics in children, were disciplined by Harvard University and its affiliated Massachusetts General Hospital.

An investigation, prompted by Sen. Charles Grassely, was conducted by Harvard University-affiliated Massachusetts General Hospital. It concluded (earlier this month) that psychiatrist Joseph Biederman and two of his proteges, Thomas Spencer and Timothy Wilens -each of who failed to disclose millions of dollars they had each received from the makers of antipsychotics, the drugs they promoted for the treatment of bipolar in children–had indeed violated the University’s/ and hospital’s conflict of interest reporting  standards.

The three wrote a mea culpa letter stating “we want to offer our sincere apologies…” acknowledging “our mistakes…”

However, no mention was made anywhere about the profound consequences of these psychiatritsts’ commercially-driven clinical recommendations. No mention about the corruption of the scientific literature, about clinical practice that deviated from the Hippocratic Oath, “First, do no harm,” nor was any mention made about the harm suffered by children whose doctors were misled about the safety and efficacy of highly toxic drugs.

Child psychiatrists and pediatricians throughout the US were guided by these exceedingly influential Harvard psychiatrists.

As Sen. Chuck Grassley noted in 2008 in the Congressional Record, “they are some of the top psychiatrists in the country, and their research is some of the most important in the field. {But] They have also taken millions of dollars from the drug companies.”

The companies that paid them millions include: Eli Lilly, Johnson & Johnson, Pfizer, GlaxoSmithKline and Bristol-Myers Squibb.

The Senator brought public attention–and to Harvard University administrators’ attention–the financial conflicts of interest, “Out of concern about the relationship between this money and their research.”

Indeed, documents uncovered during litigation confirmed that the research was scientifically corrupt and commercially-driven. The New York Times reported that Dr. Biederman promised Johnson a& Johnson that a study (yet to be conducted) in preschool children who would be given the company’s antipsychotic, Risperdal (risperidone) “will support the safety and effectiveness of Risperdal in this age group.”

“The psychiatrist, Dr. Joseph Biederman, outlined plans to test Johnson & Johnson’s drugs in presentations to company executives. One slide referred to a proposed trial in preschool children of risperidone, an antipsychotic drug made by the drug company. The trial, the slide stated, “will support the safety and effectiveness of risperidone in this age group.”

Dr. Biederman was the lead author of a trial published last year concluding that treatment with risperidone improved symptoms of attention deficit and hyperactivity disorder in bipolar children.”

Another of Biederman’s Harvard ignoble disciples was Jeff Bostic, who is also at Massachusetts General Hospital. He was named in a 2009 lawsuit joined by the US Department of Justice alleging Forest Laboratories promoted its antidepressants for pediatric use without FDA approval and paid kickbacks to docs to encourage prescriptions. He received $750,000 in payments for giving talks on using these drugs in children.

Strangely, the National Institute for Mental Health, which had awarded thse psychiatrists millions of dollars at taxpayers expense. It appears that NIMH officials did not see fit to even conduct an investigation into the corruption of science and violation of federal regulations. This demonstrates a lack of professional and moral integrity at the NIMH whose administrators think nothing about the misappropriation of public money for commercially-driven, junk research.

Harvard Docs Disciplined For Conflicts Of Interest

Three years after they were fingered in a US Senate probe into the interplay between academics who receive grant money from both pharma and the National Institutes of Health, three prominent psychiatrists from Harvard Medical School and Massachusetts General Hospital have been sanctioned for violating conflict of interest rules and failing to report the extent of their payments.

In a mea culpa addressed to their colleagues, Joseph Biederman, Thomas Spencer and Timothy Wilens wrote that “we want to offer our sincere apologies to HMS and MGH communities…We always believed we were complying in good faith with the institutional polices and our mistakes were honest ones. We now recognize that we should have devoted more time and attention to the detailed requirements of these policies and to their underlying objectives.”

And what is their punishment? They must refrain from “all industry-sponsored outside activities” for one year; for two years after the ban ends, they must obtain permission from the med school and the hospital before engaging in any of these activities and they must report back afterward; they must undergo certain training and they face delays before being considered for promotion or advancement (you can read their letter here).

The hospital had this to say: “A committee at Massachusetts General Hospital that has been looking into conflict-of-interest questions involving three MGH child psychiatrists has completed its review. Appropriate remedial actions have been taken by the hospital to address specific issues (read the statement). And a Harvard Med School spokesman sent us this: “We confirm that the review of their compliance with the Harvard Medical School Policy on Conflicts of Interest and Commitment has concluded, and appropriate actions have been taken.” He added that the conflicts policy was revised last year.

The sanctions result from a long-standing controversy over the explosive use of antipsychotics in children. Biederman, in particular (see photo), had been one of the most influential researchers in child psychiatry. Although his studies were small and often financed by drugmakers, his work helped fuel a 40-fold increase from 1994 to 2003 in the diagnosis of pediatric bipolar disorder.

For more than a decade, Biederman and his colleagues aggressively promoted the diagnosis and use of antipsychotics to treat childhood bipolar disorder, a problem that once was largely believed to be confined to adults. But the docs maintained this was underdiagnosed in kids and the meds could be used for treatment, even though they had not been approved for most pediatric use at the time. Meanwhile, the relationships with drugmakers were never properly disclosed (back story).

And for years, payments they received from drugmakers were not thoroughly reported to university officials. Yet, millions of dollars in NIH grants, which were administered by the hospital, were awarded to the docs at the same time they were receiving money from various drugmakers that make and sell antipsychotics and antidepressants. Which ones? Eli Lilly, Johnson & Johnson, Pfizer, GlaxoSmithKline and Bristol-Myers Squibb.

At one point, Biederman pushed J&J to fund a research center at MassGen that would focus on the use of its Risperdal antipsychotic in children, well before the med was approved for pediatric use. He was then placed in charge of the institute and began a study of 40 children between 4 and 6 years old who were given Risperdal and Lilly’s Zyprexa, another antipsychotic. At the time, Harvard and MGH rules forbid researchers from running trials with drugmakers if they receive more than $10,000 from a company that makes the drug (back story).

But in June 2008, US Senator Chuck Grassley made a far-reaching statement before Congress that pulled the curtain back on the money involved. The statement is memorialized in the Congressional Record. Referring to the three docs, he said “they are some of the top psychiatrists in the country, and their research is some of the most important in the field. They have also taken millions of dollars from the drug companies.”

“Out of concern about the relationship between this money and their research, I asked Harvard and Mass General Hospital last October to send me the conflict of interest forms that these doctors had submitted to their institutions. Universities often require faculty to fill these forms out so that we can know if the doctors have a conflict of interest. The forms I received were from the year 2000 to the present. Basically, these forms were a mess. My staff had a hard time figuring out which companies the doctors were consulting for and how much money they were making.”

How much were they making? At first, maybe a couple of hundred thousand dollars combined. But at his behest, the med school and hospital asked the docs to take a second look. “And this is when things got interesting. Dr. Biederman suddenly admitted to over $1.6 million dollars from the drug companies. And Dr. Spencer also admitted to over $1 million. Meanwhile, Dr. Wilens also reported over $1.6 million in payments from the drug companies.

“The question you might ask is: Why weren’t Harvard and Mass General watching over these doctors? The answer is simple: They trusted these physicians to honestly report this money.” And as Grassley then noted, there was still more money that went unreported (to read the Congressional record, click here and then check the box for 2008 and type in the name ‘Biederman’ in the search box. Men and women in the US military are more medicated than ever — and their doctors do not even know who takes what, The Daily reported Wednesday.

The Department of Defense does not keep track of medical prescriptions doled out to service members in combat, despite ongoing pleas from federal officials to do just that.

Last week, a report on the military’s 2012 budget from the House Appropriations Committee remarked that the prescription of pain management drugs is handled inconsistently, especially in battle.

The report also handed down an ultimatum: within two months of the budget’s approval, the committee wants concrete information on “the required steps and potential obstacles toward electronic transmission of prescription drug data.”

A 2010 US Army study found that 14 percent of soldiers had been prescribed an opiate painkiller, with 95 percent of those prescriptions for oxycodone, a notoriously-addictive pharmaceutical best known by the brand name OxyContin.

And since 2001, military spending on prescription medication has skyrocketed. Orders for antipsychotics like Seroquel are up 200 percent, and demand for anti-anxiety drugs like Valium has increased by 170 percent, according to Defense Logistics Agency records.

Many of the antidepressants, antipsychotic drugs and anti-anxiety drugs prescribed are highly addictive. Potential side effects include dulled reaction times, irritability and a heightened risk of suicide.

“The medications they use shouldn’t be so heavily prescribed in combat,” said Dr. Judith Broder, a psychiatrist and founder of the Soldiers Project, a nonprofit counseling service.

“But they can’t afford to send anyone home. They need the bodies — health and welfare are secondary,” she said.
